Shook Elementary Nurse

Dear Parent/Guardian,
Welcome to Shook Elementary! I would like to take the opportunity to introduce myself, and explain the role of the nurse’s office. My name is Holly Trujillo, I am a registered nurse and will be the school nurse for your student at Shook Elementary. This is my 3rd year working as a school nurse at Shook which I am very excited about. I have been an RN since 2003 working in various fields including neuro/tele and home health most recently. My office hours are Tuesday through Friday 7:30 AM to 4:00 PM.


Your student has the right to learn and my role is to advocate for your child, medically, in
the school setting. Thus, communication is key to providing a safe and high-quality education. This involves quality communication between parents, physicians, students, and school nurses. Please review your student’s health inventory and please reach out if there are any health updates, such as new health conditions, new immunizations, or new medications that we may keep note of in their medical file.

If you would like your student to have any medications at school, this includes over-the-counter medications and prescription medications, contact my office to arrange a time for dropping off the medications with the proper medication permission forms filled out. Your student is prohibited from carrying these medications on school transportation, such as the bus or on field trips without proper permission from a medical provider and has a copy of their permissions on file. They cannot be stored in lockers, backpacks, or in the classroom and must be kept in the nurse’s office where they can be properly locked and accounted for. We require parent permissions and that the medication is in its original container for labeling. It cannot be given if the label notes it is expired.

It is important to keep note of updating phone numbers and contact information of yourself and any contacts in case of an emergency or illness. Please review your information each year and contact us for changes. If your child has had a fever, vomited, or has had multiple episodes of diarrhea, they will be sent home until their illness is 24 hours symptom-free without the need of medication. This ensures the safety of our other students, staff, and your student’s health during the school year. If you have any additional questions or concerns, please feel free to contact my office.
